New York State Education Law Article 19 and Regulations of the Commissioner of Education require that students in grades Pre-Kindergarten, Kindergarten, 1, 3, 5, 7, 9, 11 and new entrants to Otselic Valley Central School receive a physical examination. Community Memorial Hospital will be available at our school on several occasions during the months of April and May to perform physicals on the students in these selected grades. The physicals are FREE and include a complete head to toe assessment.The form you will need to complete is attached via a link at the bottom of this page: "2023 Mandated physicals pdf."  Hard copies of this notification are also being sent home for your use. If your child is in 6th or 7th grade, they may require a Tanner assessment if they indicate to us that they are going to play a sport either this year or next year. If your child has already had a physical examination by their own practitioner, please have the doctor's office fax a copy to me prior to April 20, 2023. My fax number is 315-653-7267. If we do not have proof of a physical in your child's medical chart by April 20, 2023, a physical will be performed by Community Memorial Hospital at our school per New York State requirements.
