An important message from Mrs. Morse, our school nurse:
New York State requires students in grades PK, K, 1, 3, 5, 7, 9, 11 and new entrants to Otselic Valley Central School, to receive a physical examination. Community Memorial Hospital will be available at our school on several occasions to perform physicals for students in these selected grades. The physicals are free and include a complete head to toe assessment. If your child is in 6th or 7th grade, they may require a Tanner assessment if they indicate to us they are going to play a sport either this year or next year. If your child has already had a physical examination by their own practitioner, please have the doctor’s office fax a copy to me prior to April 12, 2024. Our fax number is 315-653-7267. If we do not have proof of a physical in your child’s medical chart by April 12, 2024, a physical will be performed by Community Memorial Hospital at our school. Please sign the form and return to the nurse as soon as possible.
