A time-sensitive sports physical reminder from Mrs. Morse, our school nurse:

New York State Education Law Article 19 and Regulations of the Commissioner of Education requires students in grades 7-12 to have a sports physical if they are going to play an extracurricular sport. Community Memorial Hospital is coming to our school to perform physicals in April. If you have a child in grades 6-11 and they are interested in a sport for next year, please have them sign up for a physical to be given to them. This will be the only time physicals are done this year. The physicals are FREE. If the physicals are done now, it will carry your child through every sports season next year and they will not need to have another physical until the end of the school year next year. We will NOT be doing physicals over the summer or in the fall. I strongly urge all students in grades 6-11 to ave a physical now if they think they may play a sport next year.

Please fill out, sign, and return this Interval Health History form to the nurse by APRIL 12, 2024 if you want your child to receive a physical. This is required by New York State for the health services provider to use for their assessment of your child. Any abnormal findings will be reported to you. A physical cannot be done unless the form is signed and returned.
